PCSSW Survey, Version 1, for Healthcare Administrators

Files in this grouping comprise the documents needed for a practice to conduct a patient satisfaction survey for descriptive purposes only. Benchmarking with version 1 excludes patient demographic and visit history information (see version 2 for expanded version of benchmarking).

PCSSW Administrators' Manual

A description of how to collect the survey data and how to score and evaluate the results.

PCSSW Survey

This is the National Women's Health Centers of Excellence (CoE) questionnaire without the 10 demographic questions in version 2. Version 1 must be used in its entirety without modification. Any changes will compromise the validity of the results.

PCSSW Coding Form

The acceptable responses to each question and codes used in the original CoE evaluation program.

PCSSW Data-collection Sheet

A workbook containing instructions for easily entering PCSSW data from self-report questionnaires. This utility contains "macro" programs that calculate the scale scores and summarize the results.

Tools for Researchers

PCSSW Survey, Version 2, for Health Researchers

The "Researchers Manual" is presented detailing the PCSSW psychometric properties and item and scale performance. Files in this grouping comprise the full version of the survey, which includes demographic data, to summarize the results according to different subsets of the patient population, and benchmark against CoE norms. (The WHQA will perform data benchmarking by arrangement, for a fee.)

PCSSW Researchers' Manual

Documentation of the PCSSW's development, reliability and validity.

PCSSW Survey w/ Demographics

This is the National Women's Health Centers of Excellence (CoE) questionnaire, including the ten demographic questions. Version 2 must be used in its entirety without modification. Any changes will compromise the validity of the results.

PCSSW Coding Form

A codebook for the two non-demographic sections of the questionnaire.

PCSSW Data-collection Sheet

A workbook containing instructions for easily entering PCSSW data from self-report questionnaires. This utility contains "macro" programs that calculate the scale scores and summarize the results.

Full Survey (of Interest to Health Researchers)

The complete survey used in the CoE program, including: Demographics and visit history, PCSSW, and items assessing past use of preventive and primary health care services.

The latter items are adapted from national survey tools to permit comparisons with other studies or populations. Because guidelines for care evolve over time, researchers are advised to review the item working and response to determine the extent that it reflects current recommendations for primary care or will meet the aims of the intended study.
