Secondary Appointment Requests

Faculty members with a current academic appointment at the School of Medicine may request a secondary appointment in another academic department within the institution.

  • Process: The Department Chairs of both the primary and proposed secondary departments must submit a joint letter of request addressed to the Senior Associate Dean of Faculty Affairs for approval.

Transfer of Home Department

Faculty members transferring their primary academic appointment (home department) to a new department must follow this process:

  • Process: The Department Chairs of both the current and new departments must submit a joint letter of request addressed to the Senior Associate Dean of Faculty Affairs for approval.

Wake Forest University (WFU) Joint Appointments

Faculty members with an existing appointment at Wake Forest University who wish to request a WFU joint faculty appointment must submit:

  • Current Curriculum Vitae (CV)
  • WFU Joint Faculty Candidate Summary (FCS) form

Submission:
Documents should be submitted electronically to the Office of Faculty Affairs.

Emeritus Appointment Requests

Eligibility Criteria:

  • Must be a retiring faculty member at the rank of Professor or Associate Professor.
  • Must have a record of outstanding academic contributions.
  • Must have a minimum of five years of service at the Wake Forest University School of Medicine.

Special Considerations:

  • For faculty appointed during the Atrium and Advocate integrations (Atrium, Advocate Aurora employed faculty), a combination of WFUSM and previous faculty service totaling at least five years is acceptable.
  • A detailed description of academic medicine/research service is required for these cases.

Important Notes:

  • The effective date of the Emeritus appointment may be retroactive to the retirement date.
  • Emeritus appointments do not have an end date unless one or more of the following occur:
    • Employment at WFBMC or affiliates at greater than 0.20 FTE.
    • Faculty appointment at another academic institution (excluding volunteer, adjunct, and emeritus appointments).
    • Employment at another school of medicine or medical practice.
    • Violation of Wake Forest University School of Medicine’s policies, including the Code of Conduct.

Track Change Requests

Faculty members requesting a change in faculty track must submit:

  • A letter of request, signed by both the faculty member and the Department Chair.
  • An updated Curriculum Vitae (CV).

Submission:
Request letters must be addressed to the Senior Associate Dean of Faculty Affairs for approval.
