Eligibility Requirements
- Have completed (or on track to complete) an ACGME-accredited graduate medical education program in the discipline of Internal Medicine, Family Medicine and
- Preventative Medicine
- Be board-eligible or certified by respective ABMS
- Be eligible for obtaining a full, unrestricted medical license in the State of North Carolina
- US citizen or non-citizen eligible for/with planned H-1B Visa during their fellowship

- Personal Statement or letter of interest explaining career choice and goals
- Three letters of recommendation on letterhead and signed, one of which should be from the applicant’s residency program director
- Current photograph
