MAA Distinguished Achievement Award Nomination Form

The Nominating and Awards Committee of the Medical Alumni Association invites you to submit nominees for its Distinguished Achievement Award, which is presented each year during the MAA Distinguished Awards Celebration during Alumni Weekend. If possible, please send the nominee’s C.V., other documentation of his/her accomplishments or a comprehensive letter of recommendation for review and consideration by the Committee.

Distinguished Achievement Award

Recognizes an MD alumnus or house staff alumnus who has distinguished him/herself in the profession of medicine. This individual will have made outstanding contributions through one or more of the following means: patient care, basic research, clinical research, leadership of a state or national medical organization. Consideration will be given to individuals who have achieved distinction nationally and/or locally. Additionally, he or she must demonstrate the high principles of the medical profession.
