Incontinence Impact Questionnaire and Urogenital Distress Inventory

Researchers at Wake Forest Baptist Medical Center developed and validated two survey instruments for collecting detailed data on how urinary incontinence affects women's lives. The Department of Social Sciences and Health Policy (SSHP) administers the use and permission to use the Incontinence Impact Questionnaire (IIQ) and Urogenital Distress Inventory (UDI).

The IIQ measures the impact of urinary incontinence on activities roles, and emotional states. The UDI measures how troubling the symptoms are to women.

SSHP administers the distribution and use of these two questionnaires. On request, we will send copies of the self-administered instruments (both short and long forms) and scoring materials for each instrument.

There is a one-time, $250 administrative fee for use of these questionnaires.
