Submitting a request is completely free of charge and will ensure an optimized service. It does not commit you to purchasing a questionnaire.

- If you haven't registered yet, you'll be asked to sign up for free.
- Complete the request form.

Note that users who do not receive specific funding for their use of the questionnaire can download the COAs distributed by Mapi Research Trust directly from ePROVIDE, using the “online distribution” process (see tutorial).
