The purpose of the CREATE Self Test Study is to better understand what factors impact self-testing behavior for COVID-19 and other conditions among African Americans in NC. Our primary objective is to enhance the adoption of COVID-19 self-testing, alongside testing for syndemic conditions, (HIV and diabetes), within the African American population of North Carolina. If you are participating in this study, you may have signed up at one of our Pop-Up sites or community events, or perhaps you were referred to us by another participant.

How to get started
You should have received a postcard with a QR code in your health bundle. This postcard has (1) a QR code and (2) a link to a website. Follow either of these to complete a short survey where you decide whether or not you want to participate.
Ready to submit your test results?
Upon completing the survey and agreeing to participate, expect to receive your first email requesting your test results one week later. If you've already taken your self-test, feel free to snap a photo of the completed test and save it for upload when you receive the request.
Were you recommended by someone else to participate?
Follow the link or scan the QR code on the postcard they've given you to access the survey. If you do not have a postcard, reach out to our team at create@wakehealth.edu.
